ndis services
We’re proudly registered as providers under the National Disability Insurance Scheme (NDIS).
We provide a range of services including:
- Physiotherapy
- Exercise Physiology
- Hydrotherapy/Aquatic Therapy
- Clinic-based/gym exercise programs
Our practice and bathrooms are wheelchair accessible, and our hydrotherapy pool has a pool lift for easy access in and out of the pool. All of our staff have had previous experience with different types of disability.
IMPORTANT:
For PLAN MANAGED participants: Please send us a copy of your current plan, your NDIS reference number and your Plan Manager’s details for claiming purposes. You can email this information to reception@everybodyphysio.com.au.
For SELF MANAGED participants: Payment is required upfront and an invoice can be provided for claiming purposes.
For all participants: Please download and fill out this referral form below, prior to your appointment. We must receive this form before your initial appointment or the appointment may be cancelled.
